Birth, Death & Marriage
The Register of Deeds office issues marriage licenses and is responsible for managing
and issuing copies of birth, death and marriage records that occur in their county.
The office in Forsyth County manages records only for events that took place in
Forsyth County.
Copies may be requested by mail or in person. If it's convenient, come to our office
in the Forsyth County Government Center. 201 N Chestnut Street, Winston Salem. Our
hours are Monday through Friday 8:00 am - 5:00 pm.
Walk-in customers must present valid identification for certified copies. Mail requests
for certified copies must be notarized. Uncertified copies do not require notarization.
We accept CASH ONLY at our office. For requests by mail, payment must be by money
order or certified check. We do not accept personal checks or debit/credit cards.

Customers requiring information on events that took place in other counties, states
and countries must contact the agency in which the event occurred. Each is governed
by specific statues.

Marriage Licenses
A marriage license may be obtained from the Register of Deeds office in any county
in North Carolina. The license is good for 60 days from the date of issue and a
ceremony can be held in any location within North Carolina. After the ceremony,
the license must be returned to the county that issued the license. For details,
