GOT PLANS: ADVANCE CARE PLANNING WORKSHOPS CAN HELP YOU BE PREPARED

Do you have a health care power of attorney? Do you have a living will? Do your loved ones know how to speak for you if you are ever unable to make your own medical decisions? It’s important to have discussions with family, friends and health care providers and to execute written advance directives in accordance with North Carolina State Laws.

If you want to put a plan in place but don’t know where to start, three Forsyth County Public Library branches are hosting Advance Care Planning Workshops in September and October.

A Trellis Supportive Care representative will discuss topics and provide documents and guidance for assigning a health care power of attorney and creating a living will. Registration is encouraged but not required.
